Declaration of Patient Rights

*

*Right to Privacy:

You have the right to every consideration of privacy concerning your medical care program. Case discussions, consultations, examinations and treatment are considered confidential and will be conducted discreetly. We may include certain limited information about you in the hospital directory while you are a patient at the hospital. This is so your family, friends and clergy can visit you in the hospital and generally know how you are doing. You may request to opt out of the hospital’s Patient Directory. Please notify the hospital’s registrar if you wish to do so.

*Right to Equality:

Every patient is entitled without discrimination to appropriate medical care. You have the right to treatment and medical services without discrimination based on race, age, religion, national origin, sex, sexual preferences, handicap, diagnosis, ability to pay, or source of payment.

*Right to Dignity:

You have a right to personal dignity and, to the extent reasonably possible, to privacy during medical treatment and other care. In addition, you have the right to have your cultural, psychosocial, spiritual, and personal values, beliefs, and preferences respected.

*Right to Safety:

You have the basic right to receive health care service in a safe medical setting as has been legally constructed in the S.A.F.G. Hospital.

*Right to Self-determination:

You have the right to receive from your physician(s), in terms you can understand, an explanation of your complete medical condition, recommended treatment, treatment risk(s), expected results and reasonable medical alternatives.

*Right to Make Decisions:

You have right to participate in a decision-making about your care.

*Right to Refuse(Rejection):

You have the right to refuse any drugs, treatment, or procedure offered by the hospital, to the extent permitted by law. A physician shall inform you of the medical consequences of such refusal.

*Right to Inspect and Copy:

You have the right to inspect and copy the medical information that may be used to make decisions about your care.
